Refer.to, LLC (“Refer.to” “we” or “us”) operates www.Refer.to and other Web sites and online properties on behalf of itself and/or in conjunction with third parties (collectively, the "Site"). Refer.to provides services, software and content to you via the Site (collectively, the “Services”). Refer.to is committed to ensuring the privacy and protection of users of the Site and Services. We collect personal information that is relevant to the purpose of the Site and Services, which is to provide an online patient referral and referral management platform for the exchange of patient referrals between healthcare providers. The following discloses our information-gathering and dissemination practices for the Site and Services.

Conditions of Use

If you decide to visit the Site and/or use the Services, your visit and use of the Site and Services and any possible dispute over privacy is subject to this Privacy Policy and our Terms of Use, including dispute resolution process, limitations on damages and governing law.

Types of Information Collected
Traffic Data

As you navigate within the Site and use the Services, we may automatically track and collect information about your computer, tablet, mobile device, or other equipment that you use to access the Site and Services. This information includes IP address, device specifications, domain server, and browser specifications. As you use our Services, we may collect information about your use of the various Services offered on the Site and which Web sites and other online properties you visit.

Personal Data

If you register as a user on the Site or for the Services, we may request that you provide us with information that personally identifies you. Personal information may include your name, e-mail, zip code, birth date, age, gender and geographic location. In some instances, we may also ask you to select a username and password or PIN. Additionally, when you access third-party content and/or services through the Site, you may be asked to provide personal information to a third-party provider of the content and/or services. That information is subject to the privacy policy of the third-party provider, and this privacy policy does not apply to the collection or treatment of such information via the Site or Services. For avoidance of doubt, your user name and screen name are not considered personally identifiable information and are therefore not subject to this Privacy Policy.

Children

You must be 18 years or older to use the Site and Services. We do not specifically collect information about children. If you do not certify that you are at least 18 years or older upon registration for any area or feature of the Site or Services that requires a user account or password, we will disable the ability for you to create an account.

Storage of Your Personally Identifiable Information

Refer.to utilizes third party servers for the storage of personal data and related information. The information you provide using the Site and services may be stored on servers located in states throughout the United States and internationally. We may also from time to time transmit and/or store data using cloud computing services.

Use of Your Personally Identifiable Information

We do not trade, rent, or share your personally identifiable information with third parties without your express written consent, except as specifically set forth in this Privacy Policy.

Traffic data and personal data may be used by Refer.to on an aggregate (i.e. group) basis for editorial and feedback purposes, for marketing and promotional purposes, for statistical analysis of users' behavior, for product development, for Site and Services improvement, and to inform advertisers of how many visitors have seen or clicked on their advertisements or viewed their promotions. In these cases, data is maintained and reviewed on an aggregate basis. We may share this information with our customers and other parties, but when we do so it will not include personally identifying information. In addition, we use traffic and personal data to customize and tailor your experience on the Site and Services, to help us display content and provide features and functionality that we think you may be interested in.

We may also disclose your traffic data and personal data to third parties, without further notice to you, when we believe in good faith that such disclosure is appropriate to: protect the rights, property or safety of Refer.to, the Site, the Services, our users or others; enforce our terms and conditions of usage; provide the Services; comply with law, a court order or other legal process; as requested by any regulatory agency including state and federal administrative entities having jurisdiction over the subject matter; in the course of responding to any claim by a third party that you are using Services in connection with an infringing, illegal or improper activity or that data or content you provided to the Site or used in connection with the Services infringes rights of the third party; in response to a request for identification in connection with a claim of copyright or trademark infringement by you; and to facilitate the maintenance or hosting of the Site and Services by an affiliate or service provider engaged to do so, subject to that party's agreement to comply with this Privacy Policy.

Cookies

Your browser may accept small identifying files (“cookies”) from our system that are placed on your computer. We may use cookies to keep track of your use of our Services; make sure you don't see the same ad repeatedly; deliver content, features and functionality that is specific to your interests; save your password so you don't have to re-enter it each time you visit our Site or use our Services; and for other lawful purposes which comply with this Privacy Policy. Your browser may decline the cookie if it is set to do so. Declining cookies or receiving “do not track” signals from your browser will limit the functionality of the Site and Services.

Web Beacons

A “Web Beacon” is an object that is embedded in a web page that is usually invisible to the user and allows website operators to check whether a user has viewed a particular web page. Refer.to may use Web Beacons on the Website to count users who have visited particular pages and to deliver co-branded services. Web Beacons are not used to access users’ personally identifiable information; they are a technique Refer.to may use to compile aggregated statistics about Site usage. Web Beacons collect only a limited set of information including a cookie number, time and date of a page view, and a description of the page on which the Web Beacon resides.

You may not decline Web Beacons. However, they can be rendered ineffective by declining all cookies or modifying your browser setting to notify you each time a cookie is tendered and permit you to accept or decline cookies on an individual basis. Third parties are not permitted to use Web Beacons on the Site.

Google Analytics

The Site may use Google Analytics to help analyze how users use the Site. The tool uses cookies to collect standard Internet log information and visitor behavior information in an anonymous form. The information generated by the cookie about your use of the website (including IP Address) is transmitted to Google. This information is then used to evaluate visitors’ use of the Site and to compile statistical reports on website activity for Refer.to.

We will never (and will not allow any third party to) use the statistical analytics tool to track or to collect any personally identifiable information of visitors to the Site. Google will not associate your IP address with any other data held by Google. Neither we nor Google will link, or seek to link, an IP address with the identity of a computer user. We will not associate any data gathered from the Site or the Services with any personally identifiable information from any source, unless you explicitly submit that information via a fill-in form on our website.

The Site and Services may contain links to Web sites operated by third parties, including third-party sites where you may obtain information about or access certain content, products or services. These third parties may collect personal information from you in connection with such content, products and services. We are not responsible for the content or privacy practices of any such sites. You should review any third party privacy policy or terms that govern your use of such sites or applications.

Public Forums and User-Submitted Content

The Site and Services may include the ability to post, share and edit user-submitted content in a public forum. Please remember that any information that you disclose in a publicly available area of the Site will become public information, available for use and disclosure by all users and not subject to the terms of this Privacy Policy. We do not intend to monitor, moderate, screen or otherwise be responsible for the content of postings or other user-submitted content or their subsequent use or disclosure. You should exercise caution when deciding to disclose your personal information via features or forums on our Site and in connection with the Services.

Security

The Site and Services have security measures to protect against the loss, misuse and alteration of the information within our control. We have put in place reasonable and appropriate administrative, technical, and physical safeguards and procedures to maintain the security of the information we collect and the information that is transmitted by users through use of the Site and/or Services. These measures, like any information security effort, may be circumvented or hacked, and we do not guarantee security.

You must notify us promptly if any username or password that you use on our Site or in connection with our Services is lost, stolen or used without permission. Upon notice, we will cancel that user name or password and update our records accordingly.
