Refer.to, LLC (“Refer.to” “we” or “us”) operates www.Refer.to and other Web sites and online properties on behalf of itself and/or in conjunction with third parties (collectively, the "Site"). Refer.to provides services, software and content to you via the Site (collectively, the “Services”). Refer.to is committed to ensuring the privacy and protection of users of the Site and Services. We collect personal information that is relevant to the purpose of the Site and Services, which is to provide an online patient referral and referral management platform for the exchange of patient referrals between healthcare providers. The following discloses our information-gathering and dissemination practices for the Site and Services.

Types of Information Collected
As you navigate within the Site and use the Services, we may automatically track and collect information about your computer, tablet, mobile device, or other equipment that you use to access the Site and Services. This information includes IP address, device specifications, domain server, and browser specifications. As you use our Services, we may collect information about your use of the various Services offered on the Site and which Web sites and other online properties you visit.
You must be 18 years or older to use the Site and Services. We do not specifically collect information about children. If you do not certify that you are at least 18 years or older upon registration for any area or feature of the Site or Services that requires a user account or password, we will disable the ability for you to create an account.
Storage of Your Personally Identifiable Information
Refer.to utilizes third party servers for the storage of personal data and related information. The information you provide using the Site and services may be stored on servers located in states throughout the United States and internationally. We may also from time to time transmit and/or store data using cloud computing services.
Use of Your Personally Identifiable Information
Traffic data and personal data may be used by Refer.to on an aggregate (i.e. group) basis for editorial and feedback purposes, for marketing and promotional purposes, for statistical analysis of users' behavior, for product development, for Site and Services improvement, and to inform advertisers of how many visitors have seen or clicked on their advertisements or viewed their promotions. In these cases, data is maintained and reviewed on an aggregate basis. We may share this information with our customers and other parties, but when we do so it will not include personally identifying information. In addition, we use traffic and personal data to customize and tailor your experience on the Site and Services, to help us display content and provide features and functionality that we think you may be interested in.
A “Web Beacon” is an object that is embedded in a web page that is usually invisible to the user and allows website operators to check whether a user has viewed a particular web page. Refer.to may use Web Beacons on the Website to count users who have visited particular pages and to deliver co-branded services. Web Beacons are not used to access users’ personally identifiable information; they are a technique Refer.to may use to compile aggregated statistics about Site usage. Web Beacons collect only a limited set of information including a cookie number, time and date of a page view, and a description of the page on which the Web Beacon resides.
You may not decline Web Beacons. However, they can be rendered ineffective by declining all cookies or modifying your browser setting to notify you each time a cookie is tendered and permit you to accept or decline cookies on an individual basis. Third parties are not permitted to use Web Beacons on the Site.
We will never (and will not allow any third party to) use the statistical analytics tool to track or to collect any personally identifiable information of visitors to the Site. Google will not associate your IP address with any other data held by Google. Neither we nor Google will link, or seek to link, an IP address with the identity of a computer user. We will not associate any data gathered from the Site or the Services with any personally identifiable information from any source, unless you explicitly submit that information via a fill-in form on our website.

Public Forums and User-Submitted Content
The Site and Services have security measures to protect against the loss, misuse and alteration of the information within our control. We have put in place reasonable and appropriate administrative, technical, and physical safeguards and procedures to maintain the security of the information we collect and the information that is transmitted by users through use of the Site and/or Services. These measures, like any information security effort, may be circumvented or hacked, and we do not guarantee security.
Lost or Stolen Information
You must notify us promptly if any username or password that you use on our Site or in connection with our Services is lost, stolen or used without permission. Upon notice, we will cancel that user name or password and update our records accordingly.
